If you are an authorized agent acting on behalf of a consumer, you must provide:

  • A written authorization signed by the consumer, or

  • A valid power of attorney document.

This proof of authorization may be submitted to us through our designated contact method after you file the request form. For verification purposes, we may also contact you or the consumer directly to confirm your authority.
